Membership Cancellation Policy
There are two ways to cancel your membership:
Option 1: Come into the store and complete the cancellation request form. A staff member will provide you with the paperwork and have you complete in store.
Option 2: Print THIS FORM. Please complete, sign, and return. This must be sent via CERTIFIED mail to the address below:

PLEASE BE AWARE THAT ALL ACCOUNTS MUST BE PAID IN FULL PRIOR TO CANCELLATION. ACCOUNTS WITH OUTSTANDING BALANCES WILL NOT BE CANCELLED.
